Workshop Self-Referral Form

If you are interested in any of our workshops please complete the form below in full.  Our workshops are offered in multiple formats including in person, virtual and hybrid platforms.  Please review the workshop calendars for upcoming workshop dates, times, and locations. For all virtual workshops you will require access to the internet, a personal email and computer, tablet or smartphone. You must be a resident of Durham Region and 18 years of age or older to attend any workshop. If you have any concerns or questions about referrals please contact SOS-IEHSD-Workshop-Referral-Inquiry@durham.ca.

If you are a community organization and interested in bringing one of our workshops to your client group/organization please email SOS-IEHSD-Workshop-Referral-Inquiry@durham.ca and use the subject line "Community Organization Workshop Inquiry".

Referrals

Please let us know which referrals you are interested in.

You are also agreeing to communicate with the Income Employment and Homelessness Supports Division via electronic communication for the duration of the program for the purposes of program participation and workshop evaluation. If you are an Ontario Works or Ontario Disability Support Program recipient your file will be updated with confirmation of referral to support your Action Plan. To protect the confidentiality of all participants, virtual workshops are not to be recorded. Please note that all information shared in the chat feature of our virtual workshops will be kept confidential and is only intended for the purposes of the workshop. There is no retention period for the chat history, and it will be deleted immediately following the workshop. Information collected and used will be subject to and utilized in accordance with the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA). Information is kept confidential and used only for the purposes of maintaining attendance records, program delivery and enhancing program development.
