Each food vendor must submit a completed application form to Durham Region Health Department at least 30 days prior to the event and applications must be approved prior to attendance at events. Events must comply with applicable sections of the Food Premises Regulation 493/17, under the Health Protection and Promotion Act, R.S.O., 1990 as amended.
Note: Failure to receive prior approval may result in closure of premises, or other legal action if inspection reveals significant public health concerns.

Applying for multiple events: If you are serving the same food items for multiple events in the same calendar year, you can add each event to your application by clicking the "Add another event/market" button in the section below. If the food served/sold at an event will be different, please submit a new application with the food details and associated source information.
E.g.: grocery stores, food distributers, etc.

Note: All water supply lines must be made of food-grade material.
Note: It is recommended to have sanitizer test strips for sanitizer solutions containing chlorine (bleach), quaternary ammonium, or iodine. Any other sanitizing agents must have a test strip to determine the concentration of the sanitizer.

You can upload a maximum of 1 layout documents per form submission. Our application system is unable to process/accept files larger than 6MB. If the file you are attempting to send us is greater than 6MB, you will need to reduce the file size prior to uploading it through our online form.If you need to provide us with additional files or your file is too large to upload, you will have an opportunity to provide these after you have submitted this form and a public health inspector contacts you to discuss your application.

The information is collected and used for processing your application for a new community special event or farmers’ market; to ensure compliance with legal and/or regulatory requirements; for preventing, eliminating and/or decreasing the effects of a health hazard; and, for planning, providing, and evaluating services provided by Health Department staff for promoting health and protection.
