Pharmacy Temperature Log Submission

Temperature logs are to be submitted to the Health Department the first Wednesday of each month, unless otherwise directed by Health Department staff.

In the event of a cold chain incident (temperatures outside +2.0°C to +8.0°C), contact Durham Region Health Department immediately at 905-668-7711 ext. 3063.

Reporting Pharmacy Information

Please note: You can upload a maximum of two (2) temperature log files, with a maximum file size of 7MB each, per form submission. Please consider combining your files if you require additional uploads. If you need to submit multiple temperature log files you will need to browse and upload each file separately. Our system does not allow you to select more than one file per upload.
