If you have already submitted a request for trial, do not submit another request. Instead, please call the court office at 905-668-3130 or toll-free at 1-866-779-8839 to confirm that your original request was received.

This form can only be filed for tickets issued in the Durham Region with ICON code 2860.

This form is not to be used for parking tickets. If you received a parking ticket and want to request a trial, you must follow the directions on the back of your ticket on how to submit your request.

*Note: If it has been over 15 days since you received your ticket(s), please check the status of your ticket(s) at www.ontario.ca/ticketsandfines prior to submitting your request as you may have already been deemed not to dispute the charge. If you have already been deemed, the court office cannot process your request.
