The Health Department has developed a short survey to help us learn more about the people using our online prenatal classes. The survey will take approximately one to two minutes to complete, and the answers provided will contain no identifying information and will remain anonymous. Only public health staff involved in the evaluation of the online prenatal classes will have access to your answers. Completion of the survey is optional, and you can skip any questions that you choose not to answer. Any services you receive from the Health Department will not be affected by your responses.
